Starling Nexus is your new destination for all medical oncology prior authorization requests. This proprietary technology from Starling Oncology integrates clinical decision support and utilization management into a single workflow. A simple interface lets external providers and support staff navigate a streamlined process from initial sign-in to final document submission for prior authorizations. The system consolidates what used to require multiple logins into one centralized location. 

While the platform is currently geared toward prior authorizations, this evolving hub will eventually integrate additional related services, such as pharmacy and clinical trials, so you can do more in one place.

Automatic Approval Capabilities

When providers submit orders that follow these recommended pathways, including supporting drugs, the system automatically approves the request. This feature eliminates the waiting period for standard peer reviews. Patients gain access to therapies faster, enabling them to begin lifesaving chemotherapy. These automatic approvals also save time for staff who might otherwise spend hours completing documentation, submitting paperwork, and managing phone calls. 

Clinical Precision

Before recommending pathways, Starling Nexus requires a comprehensive clinical workup, including:

  • Disease group and specific diagnosis
  • Line of therapy
  • Eastern Cooperative Oncology Group (ECOG) Performance Status
  • Cancer stage and tumor, node, metastasis (TNM) validation
  • Treatment intent

The platform also automatically calculates a patient’s body surface area (BSA). It uses all of these details to recommend a treatment pathway, enabling clinically sound, genuinely appropriate options for each patient’s specific presentation. These details also support NCCN-compliant approvals. 

Custom Regimen Capabilities

While our platform is built to auto-approve standard pathways, we understand that cancer and its treatment aren’t one-size-fits-all. When a recommended pathway doesn’t fit a patient’s needs, providers can build a custom regimen from scratch. You can search by JCode or description for necessary drugs, and the system will automatically fill in the unit and route. You can also configure the exact treatment schedule, including the total number of cycles, cycles requiring authorization, doses per cycle, service date, and place of service.

After building the core chemotherapy regimen, providers can manually attach all necessary hydration and premedications. Once you complete your order, the system automatically routes it to a Medical Director for peer review. This process ensures that even custom regimens get approvals within 48 to 72 hours.
