What Is Starling Nexus, and How Does It Work?

Starling Nexus™

Starling Nexus is our new secure, single-sign-on online portal. It enables healthcare providers and their staff to easily submit and track prior authorization requests for medical oncology, specifically chemotherapy regimens. It works by bringing prior authorization processes together into a single workflow designed for oncology. Providers and their teams can use it to streamline the treatment approval process.

What Are the Current Capabilities of Starling Nexus?

Starling Nexus is available to use as of August 17, 2026, with a range of existing capabilities and new ones coming soon. Currently, Starling Nexus exclusively handles medical oncology prior authorization requests. The platform will later include pharmacy services and clinical trials, but these are not part of it now. For radiation oncology and office visit or administrative code requests, you will continue to use Quick Cap.

Here’s what Starling Nexus does today:

  • Automating approvals: We built the system around our preferred, evidence-based treatment pathways. When providers submit treatment requests that match the recommended pathways, the system will automatically approve them.
  • Routing custom requests: When patients need custom treatment plans or providers need to add new drugs to standard pathways, the platform will automatically route the request to a Medical Director for clinical review.

Starling Nexus is designed to provide quicker turnarounds. On custom requests that need manual review, you get a decision in 48 to 72 hours. Urgent requests get a decision within 24 hours. While you’re waiting on decisions, you can check a central dashboard to see the progress of your submissions without having to call an insurance company.

Once I’m In, How Do I Locate Patients and Providers?

Once you’ve signed up, you can begin finding patients and providers. Here’s how:

Search for a patient using their member ID, or by entering a combination of their first name, last name, and date of birth. If they don’t show up, double-check the details entered and ensure they have active eligibility under the correct health plan. If they still don’t appear, you can contact PNS Support and provide the Member ID, health plan, and eligibility information.

Search by name, NPI, or Provider ID. You can only select requesting providers affiliated with your organization. For referring providers, you can search the broader network and filter by specialty if needed. If the referring provider is the same as the requesting provider (a self-referral), check the “Same as Requesting Provider” box.

If you can’t find a provider, contact PNS Support. Include the organization name, provider name, NPI, tax ID, and health plan in your request.

How Do I Build and Submit an Order?

Start by gathering the patient’s key identifying information, diagnosis codes, clinical documents, and regimen details. Then, you can follow these steps:

  1. Enter clinical information
  2. Set priority to Routine or Urgent.
  3. Select a recommended pathway or build a custom regimen.
  4. Add supportive drugs, like hydration or pre-meds.
  5. Attach at least one supporting clinical document and select its category.
  6. Click Submit Order, after which the request is sent to Quick Cap for adjudication.

What’s Next for Starling Nexus?

We eventually plan to integrate ancillary services to make Starling Nexus a hub to support your patient care. Starling Nexus will include pharmacy services and clinical trials in the future. However, these features are in development and not currently part of the platform’s capabilities. Our team at Starling Oncology will announce these capabilities as they become available, so you can begin to use them.
